To assure compliance with the energy reduction called for in Executive Order 05-16, Energy Management Services is providing State facilities with a means to report energy reduction data.

The Data Reporting Forms available below may be submitted to us either electronically (Electronic Forms) or by fax (Spreadsheet Forms).

Required reporting items:

You will be asked to perform the following tasks to fill out the report forms listed below:

Identify utility accounts
Identify buildings
Assign buildings to utility accounts
List the general use of buildings
List the square foot area of buildings
Provide base year (calendar 2005) consumption by month
Account for changes in buildings or building use through 2005 and 2006
Provide target year (calendar 2006) consumption by month

Important note: You will need Adobe® Acrobat® Reader® Version 7 to complete and electronically submit the forms referred to below. Please use the forms listed in this section to report your facility's energy usage. With Adobe® Acrobat® Reader® Version 7 (download using the link above) installed on your computer, you can submit the forms to us electronically.

Spreadsheet Forms

Please note: If Adobe® Acrobat® Reader® Version 7 is not available to you, you may complete the forms listed below by using a spreadsheet format. You will need to have Microsoft XL® software installed on your computer to complete these spreadsheet forms.

Please use the forms listed in this section to report your facility's energy usage. Complete each form using Microsoft XL®. Upon completion, please submit the forms to us by fax.
