Workers' Compensation
The State Workers' Compensation Program covers approximately 54,000 employees of the executive, legislative and judicial branches of state government, along with employees of some quasi-state agencies such as the State Fair and the Minnesota Historical Society. The self-insured, self-administered program provides and coordinates claims management, disability management and managed health care for state workers who are injured on the job and offers workers' compensation legal services for state agencies.
PLEASE NOTE: State agency use only - The First Report of Injury Form and related instructions available on this website is ONLY for Minnesota state agencies for documenting job-related illness/injuries sustained by state employees.
All other Minnesota employers should refer to the Minnesota Department of Labor and Industry and its forms when documenting employee illness/injury.
Filing a workers' compensation claim
State agencies should file a workers' compensation claim by using the workers' compensation forms and checklists.
Agency Workers' Compensation Coordinators
The Workers' Compensation Program provides many resources to assist state agencies with managing work-related injuries and illnesses. Individual agencies are encouraged to identify a primary contact as the Agency Workers' Compensation Coordinator.

Help for supervisors
Supervisors responding to a report of an injury or illness by an employee should refer to their agency's policy/ procedures and notify their Workers' Compensation Coordinator. Supervisors can also utilize the Supervisor’s Injury/Illness/Incident Reporting & Workers’ Compensation Checklist. CorVel is the certified managed health care plan for all state employees for work-related injuries and illnesses. Employees may call 612/436-2542 or 866/399-8541 to speak with a nurse about their work-related injuries and illness and to receive information about CorVel's provider network.
